2. Advice and Support
The Independent Advocacy Service is run by Voiceability and provides a district wide service for the provision of statutory independent advocacy services. This can be accessed by health and social care professionals as well as self-referrals. The service can also accept referrals for non-statutory advocacy where it has capacity to do so once statutory advocacy obligations are met.

3. Daytime Activities
Our aim is for the people we support to be happy, healthy and at home, living the life they choose as active members of their communities; each with their own aspirations and contributions to make with support when they need it to live their lives.
We have a range of Daytime Activity providers to ensure a wide range of opportunities for people with eligible social care needs to get support to:
- Get a job
- Access volunteering opportunities
- Take part in group activities
- Go out and about in a group

4. Early Help and Prevention
Early Help and Prevention services aim to support people regain indepence to the point that they no longer need support from adult social care services. These services provide personalised support early within someone's care journey with the hopes of preventing any further support or, at the very least, delay and reducing someone's need for more complex care later in their lives.
Services within this area include, but are not limited to; Alternatives to Respite, Bradford Buddy Scheme and User Led Organisations.

5. Extra Care
Extra care developments provide a focus for integrated working to meet housing, health and social care needs. They comprise of self-contained homes with design features and support services available to enable self-care and to support people to live as independently as possible within their community.
- Self-contained properties including bedroom(s), living room, kitchen and wet room
- On-site care and support staff supporting individuals to retain and regain independence through assistance with providing personal care support and guidance
- Emergency call system throughout the scheme with 24-hour support being available
- Communal facilities and services such as restaurant, lounges, hair salon and barber, and landscaped gardens
- Easily accessible via public transport routes
Varying levels of care and support needs can be provided following assessment and homes can be rented from housing associations.

6. Home Support (Home Care)
Home support services aim to support people to maintain independence in their own home by providing assistance with personal care and support, meal preparation, medication support and daily living tasks. The services generally operate between the hours of 7am to 10pm, with some assistance provided during the night. During the main operating hours people can receive up to four calls a day, which will be set out in a support plan and can be subject to change depending on their level of need.

7. Housing Related Support
Housing related support services help people to live independently or move on to independent living, this can include people receiving support to:
- Set up and maintain their home
- Manage safety and security of their accommodation
- Maintain their health and wellbeing
- Develop life skills, such as cooking, cleaning and budgeting skills
- Apply for welfare benefits
- Access other support services
- Get advice and advocacy in connection with housing or tenancy matters
Housing Related Support is delivered by a range of organisations who offer support in different settings, some of which may be in shared accommodation (such as a hostel) or may be offered in accommodation in the local community (which may be referred to as a “dispersed unit”).

8. Residential or Nursing Home (Care Home)
Residential or nursing care homes may be the right option for people whose needs are such that they can no longer be supported in their own home, in supported housing or in extra care housing and who require 24-hour care to be available.
Residential care homes provide care for people who need help with daily activities such as washing, dressing, eating and help with day to day activities.
Nursing homes accommodate those who have general care needs and also require supervision from qualified nurses. This can include those with specific or complex health needs.

9. Supported Living
Living independently means people having the right to choose the way they live their life. It means the right to receive the assistance and support they need, so they can participate in their community and live the life they want. We want to work with providers that will uphold and promote these rights in a way that is personalised and flexible.
The service will focus on the principles of maximising and sustaining independent living, choice, involvement and inclusion through person centred approaches that help people to live ordinary lives within a community setting.
