Supported Lives is part of the Potens Group, who deliver health & social care support services for children and adults with disabilities and complex needs nationwide.

Supported Lives offers support both in and outside of their home, putting together individualised packages of support to best meet each person’s needs.

This could include encouragement or support to help the individual get ready for their day, manage their medication, assist with their personal care or nutritional needs, support to enable them to attend or take part in social / educational activities or support to play a fuller part in their family life and community.
This could also be a much more intensive package of support for people who have high-level support needs, or needing more than 1 person to safely support them in achieving their outcomes.

We support people in their own homes, ranging a couple of hours a week to manage their home, right up to supporting people 24 hours a day, 365 days of the year, with an assessed and individually designed package of support, aimed at promote independence and inclusion in their local community.

Supported Lives offers an alternative to traditional day service in which each individual member has a programme of activity that best meets their needs and wants.
