 Community Integrated Care
 currently has two Supported Living services in Bradford. One of these is located in Clayton, with two bedrooms, and our second is in Low Moor, which is 27 self-contained flats.
 
 In Clayton, we provide full 1-1 and Core support, whilst in Low Moor we provide support with both Core/Waking night and personal 1-1 hours. Our Low Moor service allows for people to choose their own provider if they wish for their 1-1 hours.
 
 Our services are tailored to meet the unique needs of every person we support. We can provide as much (or as little) support as needed, from 24-hour support to just a few hours of flexible support in the community.
 
 We work closely with our housing providers who complete repairs and building maintenance when necessary.

 Our Low Moor service offers self-contained flats, enabling the people we support to be independent and live a life they choose.
 
 Some of our flats have two bedrooms too, so if someone requires extra support overnight from a member of the team, we’re here if you need it.
 
 Our Low Moor service also has a 24-hour staff office, giving people the reassurance that we’re on hand should they need it. Our onsite Hub also provides emotional support and sign-posting, enabling people who have smaller packages of support to socialise and get support when they need it.
