Working Together in Preparation for Adulthood.
Leaving school, is one of the biggest steps our children are going to take and as parent/carers we can use all of our years of lived experience to support those we care for to prepare for a good life in the future however it is how we support young people to prepare for 'adult life' as it is a life transforming time, for both the young person with SEND and their paret/carer. Having high aspirations is crucial, if a young person is to achieve the best outcomes for them to live a good and fulfilling life as an adult, starting those conversations, good communication, building working relationships and planning as early as possible is so important, be mindful Preparation for Adulthood (previously known as 'transition') starts from Year 9 (14yrs).SNOOP's Prearation for Adulthood Sessions are for young people with learning disabilities and/or autism, these sessions are focused around the 4 preparation for adulthood pathways; Independent Me, Healthy Me, Social Me and Purpose Me. Each session is person centred and planned around the pathway at an appropirate level for the individual. We peel back the complex layers of what independence, being healthy, having a social life and purpose to each day means specifically to the individual. There are many transitions throughtout life, so even if your young person has left education, these sessions can help them to continue to learn and continue to plan for life as an adult. This is an adaptive curriculum used in small groups of young people aged 14 upwards.
SUPPORT PLANNING FOR PARENTS/CARERS: Alongside this we run support planning sessions for the parent/carers, our services give you the chance to continue in employment, access training or time to do the things you want to do. You will have a support plan at the end that will be owned by you and the person you care for.
We facilitate carers meetings, with a purpose to tackle the issues that matter to you. We can offer support on special educational needs, social services issues and can attend your young person’s reviews.
We can work with you to find a way forward and point you in the right direction for your specific needs.
