We support adults aged 18+ with low-support learning disabilities, stable mental health conditions, and individuals transitioning from residential or hospital settings who can live semi-independently.

How can you access this service?

The service can be accessed through referrals from Local Authority brokerage teams, social workers, care coordinators, NHS or community professionals, with an initial review completed within 48 hours followed by assessment and placement agreement if suitable.

How long can we support you for?

Support is flexible and long-term, continuing for as long as the individual meets the criteria and requires enablement and tenancy sustainment support, with regular reviews to assess progress and ongoing need.

The service operates from a spacious five-bedroom property featuring single occupancy rooms, shared bathrooms, a fully equipped communal kitchen, a comfortable shared lounge, a large basement recreational/games area, high-speed internet, secure entry and safety systems, and nearby parking, all designed to promote independence, privacy, and social interaction.
