Bradnet - Supported Living (Transition to Adulthood : Learning Disabilities / Autism / Neurodiversity)

Bradford Provider List

Bradnet is a Bradford-based registered charity, providing supported living for young people and adults with learning disabilities, autism and neurodiversity since 2005.

We specialise in supporting young people in transition to adulthood, helping them develop the skills, confidence and stability needed to live as independently as possible.

Who we support

We support young people in transition (typically aged 18–25), and adults where appropriate, who:

  • Have learning disabilities, autism or are neurodivergent
  • Are moving from family homes, education or children’s services
  • Need structured support to develop independence
  • Experience anxiety, distress or difficulty coping with change
  • May display behaviour that challenges, particularly during transition
  • Need support that is paced, predictable and confidence-building

Support can range from low-level enablement to intensive, time-limited or longer-term packages, depending on need.

What support we provide

Support is planned with the young person and focused on building independence over time. This includes support to:

  • Settle into a new home and routine
  • Develop daily living skills such as cooking, cleaning and managing a home
  • Learn how to manage time, routines and responsibilities
  • Build confidence in social situations and community settings
  • Access education, training, volunteering or employment
  • Use local services, shops and public transport
  • Manage money, budgeting and benefits
  • Look after health and wellbeing, including attending appointments
  • Communicate needs, preferences and choices
  • Reduce reliance on paid support where possible

Support is reviewed regularly and adjusted as skills develop and needs change.

How we work

  • Transition plans are co-produced with the young person
  • Families and carers are involved where appropriate
  • Support is paced carefully to reduce anxiety and avoid overwhelm
  • Staff are consistent and matched to build trust
  • Early weeks and months focus on stability and containment
  • Reviews focus on progress, confidence and readiness for next steps

We do not rush transitions. We focus on getting them right.
