What do we offer?

We provide person-centred supported living and community-based support across the Bradford District for adults with learning disabilities, physical disabilities, and sensory impairments . Our services are flexible and outcome-focused , promoting independence, choice, and wellbeing . Support includes daily living assistance, community access, health and wellbeing support, and enablement to live independently. We are a CQC-registered provider and an approved provider on the Bradford Supported Living Services Framework (Lot 1 and Lot 3), delivering high-quality care in line with Direct Payments and Personal Budgets .

Who do we support?

We support adults aged 18+ with learning disabilities , physical disabilities , and sensory impairments , including individuals with complex needs , across the Bradford District. Our support is tailored to promote independence, choice, and inclusion , enabling people to live safely and confidently within their own homes and communities.

How can you access this service?

Our services can be accessed through Bradford Council Adult Social Care , Direct Payments , or Personal Budgets , following an assessment of need. Referrals can be made by social workers , commissioning teams , or individuals and families who are eligible for funded support.

How long can we support you for?

We offer flexible support based on individual needs, ranging from short-term and transitional support to long-term, ongoing arrangements . The length of support is regularly reviewed and adjusted in line with assessed needs and personal outcomes.

What facilities do we have?

We provide support in individuals’ own homes and supported living accommodations across the Bradford District. Our services are community-based and do not operate residential facilities, enabling people to live independently while accessing tailored support in their local communities.

The service is care-managed . We provide regulated care and support services , while housing is arranged separately through housing providers or landlords. We work closely with housing partners to ensure a coordinated and person-centred approach.
