At Personalised Care Services, we aim to make a positive difference in the lives of people with mental health conditions. We use a holistic approach, working with individuals, their families, and healthcare professionals to build meaningful connections and develop positive strategies. Our goal is to help individuals successfully reintegrate into the local community. Our support is based on an integrated approach, bringing together everyone important to the individual, including health and social care professionals. This collaboration helps the person lead a happy life. We believe that people should receive appropriate support to live fulfilling lives. This includes making reasonable adjustments to reduce or prevent anti-social or “offending” behavior and providing community forensic health and care support for those who may pose a risk to themselves or others.
We offer individualized support for people who:
Have mental health conditions such as severe anxiety, depression, psychotic illnesses, or personality disorders, which may result in challenging behavior.
Display self-injurious or aggressive behavior not related to severe mental illness, including those with specific neuro-developmental syndromes that increase the likelihood of challenging behavior.
Engage in risky behaviors that may put themselves or others at risk, potentially leading to contact with the criminal justice system (e.g., fire-setting, abusive/aggressive behavior, or sexually inappropriate behavior).
Have lower-level support needs and may not be traditionally known to health and social care services, often coming from disadvantaged backgrounds (e.g., social disadvantage, substance abuse, troubled family backgrounds) and displaying behavior that may lead to contact with the criminal justice system.
Our person-centered approach encourages empowerment and well-being. We support individuals in taking personal responsibility with a primary focus on community integration. Recovery is based on social inclusion and increased involvement in meaningful local community-based activities and events.
