Our support is designed to empower service users to strive for their best and achieve their goals. Success is achieved by setting realistic, attainable, and measurable objectives that translate into positive outcomes for those we support.A significant part of our service involves directing people to advisory services, such as advocacy support. We assist individuals in securing work placements in local businesses and support their educational aspirations, such as enrolling in local colleges, evening classes, and day courses. Additionally, we introduce service users to assistive technology and emerging approaches like social prescribing.

What do we offer?

We offer highly flexible and personalised, CQC-registered supported living services with personal hours support.  We are able to provide support that is regulated care, including personal care and medication management.  Our support is tailored to individual needs and wishes, with the flexibility to adjust support levels daily if necessary.  Our services are available to but not limited to individuals with: Learning disabilities and Autism, a dual dIndividuals 18+ with a primary learning disabilites and Autism diagnosis who have eligible care needs.  Clients with additional needs are considered on a case-by-case basis.iagnosis and high complex care needs.

Who do we support?

Individuals 18+ with a primary learning disabilites and Autism diagnosis who have eligible care needs.  Clients with additional needs are considered on Referrals should be made either by clients allocated Social Worker or fellow professionals. If the client has no allocated worker please contact your relevant social worker locality team.a case-by-case basis.

How long can we support you for?

No time limit.  We support individuals with the ultimate goal to develop their skills to live independently in the community.

What facilities do we have?

We offer Supported Living Services and have close links with housing provision, we have access to Self contained Housing and communal living Nohousing.

No

How is the service managed, Housing or Care?

Commisioned Care and Support is delivered by Empowered Lives. Tenancies are held with either Bradford Supported Living and private landlords that Empowered Lives work with.

What is the size of the setting/building?

1 bed flats and houses with shared communal areas.
