Vision Care Services – Social Inclusion Support for Adults and Young People

At Vision Care Services, we believe that everyone has the right to participate fully in their community, build meaningful relationships, and enjoy purposeful, fulfilling lives. Our Social Inclusion Support is designed to reduce isolation, build confidence, and help individuals with additional needs feel connected, valued, and empowered.


Whether someone is living with a learning disability, autism, mental health challenges, or physical health conditions, we offer personalised, compassionate support to help them engage with society on their own terms. We work alongside each person to understand their interests, goals, and barriers—then co-develop support plans that promote inclusion, wellbeing, and independence.

Each person is supported by a trained, consistent support worker who gets to know them well and builds trust over time. A dedicated care coordinator oversees the package to ensure it remains responsive, safe, and person-centred.


Our Social Inclusion Support includes:

  • Encouragement and support to attend social groups, clubs, and activities

  • Help to build and maintain friendships and social networks

  • Support accessing education, volunteering, or training opportunities

  • One-to-one support with travel and accessing the community confidently

  • Accompaniment to faith-based, cultural, or recreational events

  • Building daily living and social skills (e.g., communication, budgeting, self-advocacy)

  • Structured support for those experiencing loneliness, anxiety, or low confidence

  • Helping individuals to engage with local services, groups, and peer networks


We take a strengths-based approach, helping people set personal goals—whether that’s joining a gym class, visiting a friend, learning to use public transport, or getting involved in a local project. Social inclusion is not just about being present—it’s about being part of something meaningful.

Our staff are trained in inclusive communication, positive behaviour support, and low-arousal techniques to ensure our support is always respectful, appropriate, and empowering.
