Who we Support
Aurelia are skilled in providing person centred, outcome based support for vulnerable people with learning disabilities and physical disabilities. We believe that individuals have the right to lead active and fulfilling lives, participating within their community.
We can either provide this support in peoples own home or in properties which we can provide to secure individuals their own tenancy.
Individuals are helped to maximise their abilities and opportunities to improve their quality of life. Each person is also encouraged to work towards increasing their independence in their local community.
How We Support
We provide person-centred support that is carefully planned, delivered, and reviewed to ensure each individual achieves their goals and maintains independence. Our approach includes:
-
Assessment of Need – We begin with a thorough assessment to understand individual circumstances, strengths, and aspirations.
-
Transition Planning – Support is tailored to ease transitions, whether moving into a new home or adjusting to changing needs.
-
Person-Centred Planning – Development of support, risk, and care plans are co-produced to reflect personal preferences and priorities.
-
Recruitment and Selection – We carefully match and recruit team members to ensure compatibility and quality of care.
-
Person-Centred Training – Our support teams receive bespoke training to deliver compassionate, consistent, and safe care.
-
Support with Moving – We provide practical and emotional support to move into a new home smoothly and with confidence.
-
Ongoing Tenancy Support – Individuals receive assessed support to live safely and independently in their own home.
-
Continuous Review – Regular reviews ensure that support remains responsive, effective, and aligned with changing needs.
-
Independence Goals – Plans are developed and adapted to promote independence and achieve personal aspirations.
-
Outcome Reviews – Support hours and interventions are adjusted as outcomes are met, encouraging progress and growth.
-
Expert by Experience Input – We embed lived experience by feeding into our Aurelia Team, ensuring services reflect real needs and insights.
