What do we offer?
We provide: Personal Care, Support with medication, Social Support, 24 - hour care, Shopping and domestic duties to adults with mental health needs supporting to live independently in their own homes and access the local community.
Who do we support?
Individuals 18+ with a primary mental health diagnosis with eligible care needs.
How can you access this service?
Referrals should be made by allocated Social Worker or CPN. If no allocated worker please contact your relevant community mental health team.
If you are receiving direct payments or are a self-funder you can contact us directly.
