Creative Support is on the Mental Health Supported Living provider list for CBMDC.
With over 30 years’ experience of supporting adults with Mental Health, as well as those that display behaviours that may challenge, have difficulty with substance misuse or have been in contact with the criminal justice system. By understanding support across a range of situations, we are able to work alongside people to create bespoke and personalised support offers that can work for them. We provide support to empower individuals to build and lead independent, fuller and happier lives. Our supported living helps people from all walks of life to live more independent and happy lives.

What do we offer?
Becoming part of the community is central to our support, job seeking or playing a part in local voluntary activities are some of the things that we promote and encourage. Developing relationships are supported through good and difficult times. We actively encourage the discovery of new skills and interests, supporting individuals in the community. Our Bradford Supported Living is 1-1 personal hours with shared sleeping night at the property.
Who do we support?
Individuals 18+ with a primary mental health diagnosis with eligible care needs.
How can you access this service?
Referrals should be made by allocated Social Worker or CPN. If no allocated worker please contact your relevant community mental health team.
How long can we support you for?
No time limit. We support individuals with the ultimate goal to develop their skills to live independently in the community.
What facilities do we have?
Self contained flats with own kitchen and bathroom. Our scheme of 5 self-contained apartments with communal garden offers easy access to shops, bars, cafes, 24-hour shared background support, sleep-in staff and individually agreed 1:1 personal support.

No
How is the service managed, Housing or Care?
Commisioned Care is delivered by CareTech. Our existing Bradford services would offer your accommodation with a tenancy with Halo Housing as your Landlord

5 self contained flats with shared communal area and front door entrace.
