Care Home Brokerage
Introduction
From 3rd November all new care home placements for older people will be made through the Care Home Brokerage Team. Please make sure your care home is prepared for the change.

There are actions you need to take now to prepare for this change:
- Please provide and email address for your Connect to Support Brokerage account. This is how referrals will be sent to your care home. We recommend, where possible, using a general email address (for example, info@abccare.co.uk) for your Connect to Support account. You will need a unique account for each separate care home if you are part of a group:

This form will take less than 5 minutes to complete. - Please complete the Care Home Brokerage Information Form. This form provides information about your care home, which will be used to decide what referrals to send to your home. It will be combined with information on vacancies form the Capacity Tracker and other information held by Bradford Council on your care home:

This form will take around 20 minutes to complete. - The terms and conditions of the Bradford and District Residential and Nursing Care Homes Provider List have been varied to reflect the introduction of the Care Home Brokerage Team and new referral routes. Please review the updated documents here:
